  Note: To determine if you are required to report this information on a 2017 federal income tax return, see the federal income tax instructions, or contact the IRS or your tax professional.
If you receive a message stating that you are unable to access your statement and you received a 2016 New Jersey income tax refund, please review our Frequently Asked Questions or contact the New Jersey Division of Taxation's Customer Service Center at (609)292-6400 with any inquiries from 8:30 a.m. to 4:30 p.m., Monday through Friday, excluding legal holidays.

The Division does not issue prior year 1099-G forms. If you need information from a prior year, you must contact the Division.
