Welcome to our online Nursing Home Provider Assessment Service. You can use this service to file or amend your quarterly return, make your quarterly payment or pay an outstanding liability. You may make your payment via electronic funds transfer, credit card or e-check. At the end of the process, we will provide you with a confirmation number for your records. We hope you find this service helpful and encourage you to E-MAIL us with your comments, questions and recommendations.

To use this service you must enter your Federal or New Jersey taxpayer identification number and Nursing Home Provider Assessment PIN (Personal Identification Number) or business name. To use the service to file the New Jersey Nursing Home Provider Assessment Return (NHA-100) and the New Jersey Home Provider Assessment Amended Return (NHA-100X) you will need a PIN (Personal Identification Number) number that is supplied by the Division of Taxation or simply enter your business name. In most cases you can enter the service by using any PIN assigned to your identification number. This PIN could be the PIN used to enter the Sales Tax Service, Employer Report Service or any other PIN assigned to your identification number. The exception is for businesses which have requested separate PIN numbers for each tax they are eligible for. Those businesses must utilize the tax specific PIN for Nursing Home Provider Assessment assigned to their business. The New Jersey Division of Taxation issues PIN's to qualified businesses. If you do not have a PIN, CLICK HERE to submit an electronic request to receive your PIN by e-mail. Your request should indicate that you need a PIN, the return you are attempting to file, and your Business Name and Tax Identification Number.

A return and/or payment accepted for processing by 11:59 PM on the due date will be deemed timely. If the due date falls on a weekend or a legal holiday, the return and payment are due on the following business day.

Your Taxpayer ID Number
This is a 12 digit ID number. In general, it is your 9 digit Federal Employer Identification Number (FEIN) assigned to you by the Internal Revenue Service (IRS), followed by a 3 digit suffix. If you do not have a suffix, enter 3 zeroes. Enter the 9 digit FEIN and the 3 digit suffix without dashes, slashes, or any other special characters.
(Example: If your FEIN is 12-3456789, enter 123456789000).
