You have indicated that you want to submit an Amnesty payment because you were not outreached, you are not registered with
the Division of Taxation or you wish to make a self-assessment. In addition to your Amnesty payment, all required tax returns
must be completed and filed no later than January 15, 2019. If required returns are not received before the end of the Amnesty
period, Amnesty cannot be granted. File all applicable returns electronically by 11:59 p.m. on January 15, 2019 or by mail to: Tax Amnesty Headquarters, PO Box 286, Trenton, NJ 08695-0286, postmarked by January 15, 2019.
